Research project regarding occupational therapy services being provided in high schools

My name is Amanda Hoag and I am a first-year graduate student a Bay Path College in Longmeadow, MA.  I am currently working on a research project regarding occupational therapy services being provided in high schools.  I was wondering if you provide such services, and if so, would you be willing to fill in a short survey, which should not take any more than 10 minutes of your time?  If you do not provide such services could you please pass this on to anyone you know who does and/or have them contact me at ahoag@baypath.edu so I can send them the survey.
Due to the fact this survey is being transmitted to you electronically, it is assumed that if you complete the survey you are consenting to be part of the research study.  Survey Link:
http://www.surveymonkey.com/s/CF7GWVG. Any help with this project would be extremely appreciated.  To read the actual consent form, or for questions or concerns contact Amanda Hoag at ahoag@baypath.edu
I understand there is a major concern regarding the confidentiality of the students you provide services for, so if you do not wish to provide the name and contact information of a student who has successfully transitioned into the workforce please let me know and I will send you the consent form and short survey for the students.  If you still do not wish to provide this type of information please contact ahoag@baypath.edu for a survey for therapists.
